Exhibit B —Scope of Services—Fiscal Year 2012-13 <br /> Orange County Outside Agency Performance Agreement Certification <br /> Funded Amount: $ 1,000.00 <br /> Agency: Duke HomeCare &Hospice <br /> Program <br /> Duke Hospice provides palliative care for terminally ill patients regardless of ability to pay. The <br /> Unicorn Bereavement Center in Hillsborough provides community bereavement services under <br /> the hospice umbrella. Our annual bereavement camp for kids is also located in Orange County at <br /> Camp New Hope. <br /> Purpose <br /> The requested funds will be used to provide bereavement services to the community and provide <br /> hospice care for indigent residents of Orange County. We will continue to provide quality, <br /> comprehensive hospice services for all terminally ill patients within the community regardless of <br /> ability to pay. Our bereavement services will continue to provide counseling, support-groups and <br /> workshops for everyone in the community, including the Chapel Hill-Carrboro and Orange <br /> County School Systems. <br /> Outcomes <br /> 1. 90% of families will be satisfied with the care we provide. <br /> 2. Children and caregivers will recommend Camp ReLEAF to others. <br /> 3. 95% of those who use our bereavement services will be satisfied. <br /> 4. Provide hospice care for all terminally ill patients in our service area, regardless of ability <br /> to pay. <br /> Program Budget <br /> Duke Hospice provides over$3,000 of charity care each month; your grant of$1,000 will <br /> support charity care provided in Orange County in Fiscal Year 2012-2013. <br /> Certified by: i -- c a.
