" <br /> Exhibit B—Scope of Services—Fiscal Year 2012-13 <br /> Orange County Outside Agency Performance Agreement Certification <br /> Funded Amount: $28,000 <br /> Agency: Freedom House Recovery Center, Inc. <br /> Programs: Alvis and Chapel Hill Men's Halfway Houses and Facility Based Crisis and <br /> Detox <br /> Purpose <br /> The mission of our halfway houses is to promote recovery by providing long-term,residential <br /> care and treatment in our 22 beds, for over 99 women and men, ages 18 and older, to break the <br /> cycle of poverty, addiction and illness, (1) effecting a 95%utilization rate for these programs, <br /> (2)reducing the incidents of relapse, and (3)reconnecting people to family,jobs and housing <br /> upon graduation. This long term treatment program provides the treatment and case <br /> management needed for our residents to recover from their addiction disease and mental illness, <br /> and to sustain their lives and their family's lives as they graduate. Six of the 22 beds are focused <br /> on community diversion services for Orange County's Criminal Justice Partnership Program. <br /> Outcomes <br /> Program will provide long term (3 to 6 months) recovery services and community support <br /> services to 95 men and women in need of addiction and recovery treatment during 2012/13, in <br /> order to break the cycle of addicted behavior. <br /> Outcome Objectives (Describe the result(s) the agency expects to achieve by the end of the <br /> funding cycle due to the tasks and activities performed as stated above) <br /> Outcome Objective#1: Maintain a 95%utilization rate in the halfway house programs. Staff <br /> will work with stakeholders/community partners (UNC Hospital, OPC LME, Court System, <br /> Drug Treatment Court, Family Court, Mental Health Court, IFC Shelter, etc.)to identify referrals <br /> for individuals in our community who need long-term halfway house services. <br /> Outcome Objective#2: 75% of all clients will graduate from the program and re-enter the <br /> community and/or family. Reconnect individuals to family,jobs and housing. Staff will <br /> develop a Person Centered plan, with a goal of self-sufficiency and sobriety upon graduation. <br /> Outcome Objective#3: 70% of clients will have sustained sobriety at their discharge. <br /> Outcome Objective#4: 54%of clients who graduate will have secured housing upon discharge. <br /> Outcome Objective#5: 55%of clients will find employment while in the programs. <br /> The mission of our Facility Based Crisis/Detox Program is to promote recovery by providing <br /> immediate and strong response to crises events in the lives of individuals suffering from <br /> addiction and/or mental illness. This program is generally the first step toward wellness and <br /> recovery for people, and it met the community's need for immediate response to crises for over <br /> 1,918 men and women over the age of 18 last year.
