•
Exhibit B—Scope of Services—Fiscal Year 2012-13
Orange County Outside Agency Performance Agreement Certification
Funded Amount: $20,000
Agency: Senior Care of Orange County
Program
This is a continuation of the financial support provided to the Florence Gray Soltys Adult Day
Health Program under the auspices of Senior Care of Orange County, Inc. This program
originally started by the County Dept. on Aging as a major Master Aging Plan funded initiative.
County funds are critical for the continuation of this one of a kind program that serves the
severely frail elderly in Orange County. (The program has expanded as of February 4, 2009 as a
partnership with the county to continue to work towards meeting the goals of the Master Aging
Plan of Orange County.)
Purpose
The Florence Gray Soltys Adult Day Health Program is a therapeutic health focus program for
adults and seniors offering a variety of services in a home-like setting that is open from 7:30am
to 5:30pm, Monday-Friday. The program goals are: promote personal independence and health;
maintain social, physical, and emotional well-being; provide the opportunity for adults to stay
connected with their peers and the community; and provide relief and support for family
caregivers.
What services are offered? A safe, caring environment; individual care plans addressing the
social,physical and emotional needs of the participant; assistance with or supervision of daily
activities by caring, qualified professionals; programming focusing on achievement, learning
new skills, encouraging creativity, and group interaction; USDA approved meal and snacks;
arrange door-to-door transportation; education, relief and support for the caregivers of program
participants, and staffing by certified nursing assistants, and healthcare monitoring by registered
nurses, activity coordinator and program director.
Outcomes
To prevent and or prolong 75% of participants with health conditions from becoming isolated
and institutionalized during a one-year period of time by allowing them to remain living in either
their home or the home of a close family member.
Activities: Offer ongoing, stimulating activities on a daily basis Monday through Friday between
the hours of 7:30am-5:30pm. Within this same schedule, assist with activities of daily living
(i.e. toileting, shower services and assisting with meals), manage medications (if required),
conduct a monthly physical examination and maintain individualized care plans for participants.
