• <br /> Exhibit B—Scope of Services—Fiscal Year 2012-13 <br /> Orange County Outside Agency Performance Agreement Certification <br /> Funded Amount: $20,000 <br /> Agency: Senior Care of Orange County <br /> Program <br /> This is a continuation of the financial support provided to the Florence Gray Soltys Adult Day <br /> Health Program under the auspices of Senior Care of Orange County, Inc. This program <br /> originally started by the County Dept. on Aging as a major Master Aging Plan funded initiative. <br /> County funds are critical for the continuation of this one of a kind program that serves the <br /> severely frail elderly in Orange County. (The program has expanded as of February 4, 2009 as a <br /> partnership with the county to continue to work towards meeting the goals of the Master Aging <br /> Plan of Orange County.) <br /> Purpose <br /> The Florence Gray Soltys Adult Day Health Program is a therapeutic health focus program for <br /> adults and seniors offering a variety of services in a home-like setting that is open from 7:30am <br /> to 5:30pm, Monday-Friday. The program goals are: promote personal independence and health; <br /> maintain social, physical, and emotional well-being; provide the opportunity for adults to stay <br /> connected with their peers and the community; and provide relief and support for family <br /> caregivers. <br /> What services are offered? A safe, caring environment; individual care plans addressing the <br /> social,physical and emotional needs of the participant; assistance with or supervision of daily <br /> activities by caring, qualified professionals; programming focusing on achievement, learning <br /> new skills, encouraging creativity, and group interaction; USDA approved meal and snacks; <br /> arrange door-to-door transportation; education, relief and support for the caregivers of program <br /> participants, and staffing by certified nursing assistants, and healthcare monitoring by registered <br /> nurses, activity coordinator and program director. <br /> Outcomes <br /> To prevent and or prolong 75% of participants with health conditions from becoming isolated <br /> and institutionalized during a one-year period of time by allowing them to remain living in either <br /> their home or the home of a close family member. <br /> Activities: Offer ongoing, stimulating activities on a daily basis Monday through Friday between <br /> the hours of 7:30am-5:30pm. Within this same schedule, assist with activities of daily living <br /> (i.e. toileting, shower services and assisting with meals), manage medications (if required), <br /> conduct a monthly physical examination and maintain individualized care plans for participants. <br />
