. • <br /> Exhibit B—Scope of Services—Fiscal Year 2012-13 <br /> Orange County Outside Agency Performance Agreement Certification <br /> Funded Amount: $35,000 <br /> Agency: Orange Congregations in Mission (OCIM) <br /> Program: Samaritan Relief Ministry <br /> Meals on Wheels <br /> Support Services <br /> Purpose: <br /> The Samaritan Relief Ministry (SRM) provides nutritious food and financial assistance to <br /> the economically insecure population of northern Orange County. <br /> Meals on Wheels (M0119 provides home-delivered meals to the frail and elderly in our <br /> community. <br /> Support Services (SS) administratively supports Orange Congregations in Mission. <br /> Outcomes: <br /> (SRM)One hundred percent of qualified referrals will receive up to one week of groceries a <br /> maximum of five times in a 12-month period, thus providing improved access to food for <br /> food-insecure households. <br /> One hundred percent of those receiving emergency financial assistance will be able to retain <br /> their housing at least 30 additional days. <br /> (MOW9A total of55 people with a daily average of22 people will receive a nutritious meal <br /> five days a week. These meals will be delivered by volunteers to people who are <br /> homebound, home alone and unable to prepare meals for themselves, thus allowing these <br /> individuals to stay in their homes for as long as possible. <br /> Program Budget: <br /> Samaritan Relief Ministry: 53% ($18,555) of the Orange County funding will purchase food <br /> and rental and utility assistance. <br /> Meals on Wheels:37%($12,950) of the Orange County funding will assist with subsidizing <br /> the meal cost for recipients. <br /> Support Services: 10% ($3,500) of the Orange County funding will assist with salary for the Client <br /> Services Manager. <br /> Certified byabv. kn411- jJITit1e:: FY-'-e/c.t...tu)L <br />
