Social workers and the adult service unit <br /> supervisor will contact local health agencies, attend community forums, senior fairs, community <br /> resource committee meetings speak with local doctors, hospital social workers, hospice social <br /> workers and intake workers in both the Aging Department as well the Department of Social <br /> Services. Referrals will be screened to assure the needs of low-income minority and rural elderly <br /> are addressed. This will include close collaboration with the Medicaid and Food and Nutrition <br /> units. <br />
