The <br /> community service provider shall specify how the service needs of low income, low- <br /> income (including low income minority elderly), rural elderly and elderly with <br /> limited English proficiency will be met through the services identified on the Provider <br /> Services Summary(DOA-732). This narrative shall address outreach and service <br /> delivery methodologies that will ensure that this target population is adequately served <br /> and conform with specific objectives established by the Area Agency on Aging, for <br /> providing services to low income minority individuals.
