2012-2017 Orange County Master Aging Plan <br />Long-term care facilities A variety of services which help meet both the medical and <br />non-medical needs of people with a chronic illness or <br />disability who cannot care for themselves for long periods <br />of time. <br />Mobility manager An employee who coordinates transportation services across <br />providers and consolidates information about services for <br />users.29 <br />Nursing homes Also called skilled nursing unit, skilled nursing facilities, care <br />home, or long-term care facility. Please see Long-term care <br />facility for a definition. <br />Objective Identifies how a goal will be achieved in concrete, <br />measurable terms. Objectives provide a framework for <br />evaluation. Ideally, they should be SMART (Specific, <br />Measurable, Achievable, Realistic, Time-bound). When <br />possible, objectives state who will change, by how much, by <br />when, and how the change will be measured.26 <br />PACE program PACE stands for Program of All-Inclusive Care for the <br />Elderly, and provides long-term care services and supports <br />to Medicaid and Medicare enrollees.3o <br />Palliative care An area of healthcare that focuses on alleviating and <br />preventing patient suffering, particularly for those nearing <br />the end of life. <br />Patient-centered care An approach to healthcare that serves each patient with <br />individual support, unbiased guidance, and views the <br />patient as a whole-person within the context of his or her <br />family, religion, and life history. <br />Rental assistance A federal program that provides rental assistance to low- <br />income families who are unable to afford market rents. <br />Secular chaplain A minister who offers non-denominational, religiously- <br />appropriate spiritual counseling to individuals in a secular <br />setting. <br />SeniorNet SeniorNet is a 501(c)3 nonprofit organization of computer- <br />using adults, age 50 and older. SeniorNet's mission is to <br />provide older adults education for and access to computer <br />technologies to enhance their lives and enable them to share <br />their knowledge and wisdom. <br />Scam Jams A seminar designed to educate older adults in how to <br />recognize and avoid fraud. <br />53 <br />
