GLOSSARY <br />2012-2017 Orange County Master Aging Plan <br />_...._.__........._ _ _ <br />Adult Protective Servites (APSj A program housed at all county Departments of Social <br />Services that helps older adults (age 65+) and dependent <br />adults (ages 18-65 who are disabled), when these aduits <br />are unable to meet their own needs, or are victims of abuse, <br />neglect, or exploitation.2z <br />Assisted living facilities Facilities that provide support services and supervision to <br />residents in order to ensure their safety and wellbeing. <br />Chatham-Orange CRC The Chatham-Orange Community Resource Connection <br />(CRC) is a collaboration of more than 25 public and private <br />agencies, health care providers, non-profits, and social <br />service providers. This partnership provides a one-stop-shop <br />for aging adults and adults with disabilities looking for <br />resources and information on services and supports. <br />Complete Streets policy Complete Streets policies promote transportation systems <br />that incorporate multiple modes of travel, including <br />bicycling, walking, driving, qnd using public transportation, <br />and that are safe and easy to use for people of all ages <br />and abilities. In this way, these policies support health and <br />quality of Iife.23 <br />Congregational nurses Congregational or parish nurses who serve as practicing <br />nurses within a faith community. <br />Direct care workers Professionals and paraprofessionals who provide direct <br />support and services as home health aids or to residents of <br />long-term care facilities and their families. <br />Door-through-door transportation When a rider is physicalfy assisted from within their home to <br />a vehicle, given a ride, and assisted from the vehicle into <br />their place of destination.24 <br />Door-to-door transportation When a rider is physically assisted from their door to a <br />vehicle, given a ride, and assisted from the vehicle to the <br />door of their destination.24 <br />Escorted transportation When a rider receives assistance in and out of a vehicle, <br />including help fastening seat belts, etc. Escorts may also <br />accompany a rider to their destination, such as the grocery <br />store, in addition to providing transportation.24 <br />EZ Rider This is the door-to-door Paratransit service provided by <br />Chapel Hill Transit to eligible riders with physical and~or <br />mental disabilities.25 <br />Family care homes An adult care home that provides housing and services and <br />has no more than six residents. <br />51 <br />
