1�17��1 me,nt <br /> 4 <br /> Community Based Public Health Initiative <br /> Grant Project Ordinance <br /> Program Number 11-644 <br /> Be it ordained by the Orange County Board of County Commissioners that pursuant to Section <br /> 13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is <br /> hereby adopted. <br /> Section 1.The project authorized is the Community Health Advocacy Program (CHAP) as <br /> awarded to the Orange County Health Department by the Kellogg Foundation. The <br /> purpose of this four-year grant is to improve the health of minority populations in <br /> targeted communities. <br /> Section 2.The officers of the County are hereby directed to proceed with the grant project within <br /> the grant document and the rules and regulations of the Kellogg Foundation. <br /> Section 3.The following revenue is anticipated to be available to complete this project: <br /> 1993-94 Grant Funds - CHAP $93,884 <br /> 1994-95 Grant Funds - CHAP $93,626 <br /> 1995-96 Grant Funds - CHAP $82,160 <br /> 1996-97 Grant Funds - CHAP $33,750 <br /> 1997-98 Grant Funds - CHAP $11,250 <br /> 1998-99 Grant Funds -Community Health Initiative $30,028 <br /> 1999-00 Grant Funds - Community Health Initiative $20,463 <br /> 2000-01 Grant Funds - Community Health Initiative $46,717 <br /> 2001-02 Grant Funds- Community Health Initiative $49,056 <br /> 2002-03 Grant Funds - Community Health Initiative $46,575 <br /> Total CHAP Grant Funds $560,329 <br /> Section 4.The following amount is appropriated for this project: <br /> Human Services- Community Based Public Health Initiative $560,329 <br /> Section 5. The finance officer is hereby directed to maintain within the Grant Project sufficient <br /> specific detailed accounting records to provide the accounting to the grantor agency <br /> required by the grant agreement and federal and state regulations. <br /> Section 6. Funds may be advanced from the General Fund for the purpose of making <br /> payments due. <br /> Reimbursement requests should be made to the grantor agency in an orderly and <br /> timely manner. <br /> Section 7. Copies of this grant project ordinance shall be made available to the finance officer <br /> for direction in carrying out this project. <br />
