Community Health Assessment <br /> Introduction to Community Health Assessment <br /> The 2011 Community Health Assessment(CHA) is intended m <br /> to enable local public health officials and community groups <br /> to monitor trends in health status, identify priorities among <br /> health issues, and determine the availability of resources <br /> within Orange County to adequately address these priorities. <br /> The document seeks to be useful, relevant,actionable,and <br /> both reflective and forward-looking;and to provide ' <br /> information for effective strategic community health planning. 5 E9W y" <br /> The Orange County Health Department,and the Healthys <br /> Carolinians of Orange County(HCOC) Partnership with its 125 <br /> individual members from 80 partner agencies and community <br /> representatives,worked collaboratively to complete the <br /> community health assessment.
