Agreement between <br /> UNC Family Practice <br /> And <br /> Orange County Sheriffs Office in collaboration with <br /> Orange County Health Department <br /> Whereas the Orange County Health Department and the Orange County Sheriffs <br /> Office are both departments within the County of Orange; and <br /> Whereas the Orange County Health Department provides Jail Health services for <br /> the Orange County Sheriffs Office; <br /> And whereas the Health Department has need for the provision of after-hours <br /> medical consultation for the jail; <br /> And whereas UNC Family Practice is willing to provide such services; <br /> Therefore the parties agree to the following: <br /> UNC Family Practice shall: <br /> • Provide medical and pharmaceutical consultation and guidance to jail <br /> staff by telephone as requested between the hours of 4:30 PM and 7:30 <br /> AM Monday thru Friday,weekends(4:30 PM Friday through 7:30 AM <br /> Monday), and holidays. <br /> • Document consultation or guidance provided and make available via fax or <br /> e-mail the next day to Jail Health staff,as requested,to assure continuity <br /> of care. <br /> • Invoice quarterly the Orange County Sheriffs Office for service rendered In <br /> accordance with this agreement. <br /> Orange County Sheriffs Office in collaboration with the Orange County Health <br /> Department shall: <br /> • Reimburse UNC Family Practice at the rate of$500 per month($1500 per <br /> quarter)for the above after-hours medical consultation. <br /> • Review Invoice and make payment for service rendered In accordance with <br /> this agreement. <br /> This agreement shall be In effect from July 1,2002 through June 30,2003. <br />
