A .~ V ~ <br />STATE OF NORTH CAROLINA UNCH#64C~ <br />COUNTY OF ORANGE <br />AGREEMENT BETWEEN <br />THE LJNIVERSITY OF NORTH CAROLINA HOSPITALS <br />AND <br />ORANGE COUNTY <br />THIS AGREEMENT, made effective the 1 st day of July, 1997 by and between <br />The University of North Carolina Hospitals, hereinafter referred to as "The Hospital," arad <br />Orange County, for and on behalf of the Department of Aging, hereinafter referred to as <br />"The County". <br />WITNES SETH: <br />WHEREAS, The County desires to provide the services of a full-time Wellness <br />Program Director to work with The Hospital to expand e~cisting programs and to develop <br />and implement new programs to serve the health and wellness needs of seniors in Orange <br />County, North Carolina; and <br />WHEREAS, The Hospital desires to provide financial and other resources to The <br />County in support of existing and new senior health and wellness programs under the <br />terms and conditions set forth in this Agreement. <br />NOW, THEREFORE, in consideration of the premises and of the following mutual <br />promises, covenants, and conditions, The Hospital and The County agree as follows: <br />1. The County will provide the following: <br />a. The services of a full-time Wellness Program Coordinator who will have <br />the following responsibilities: <br />(1) Prepare an annual Otder Adult Wellness Plan ("Wellness Plan") <br />with the assistance ofthe Weliness staffteam submitted to the Advisory Committee for <br />approval. <br />(2) Implement the Wellness Plan which includes the coordination of <br />community-based partnership resources and the direct provision of services offered at <br />Seruor Centers. ~ <br />(3) Promote the increased involvement of older adults in the <br />Wellness Program with the assistance of community-based partnerships. <br />(4) Evaluate the impact of the Wellness Program initiatives on older <br />adults by the establishment of assessment procedures. . <br />
