ORANGE COUNTY <br />BOARD OF COMMISSIONERS <br />ACTION AGENDA ITEM ABSTRACT <br />Meeting Date: February 7, 2012 <br />Action Agenda <br />Item No. 5- k <br />SUBJECT: Intergovernmental Agreement <br />DEPARTMENT: Health Department <br />County Attorney <br />PUBLIC HEARING: (Y/N) No <br />ATTACHMENT(S): <br />INFORMATION CONTACT: <br />County Attorney's Office- 245-2317 <br />Colleen Bridger -Health Director <br />245-2812 <br />PURPOSE: To approve an intergovernmental agreement. <br />BACKGROUND: The Board directed that it review all intergovernmental agreements. Provided <br />below is an Agreement and brief synopsis that has been reviewed by the County Attorney's <br />Office and staff is asking the Board to approve. <br />1. Parties to the Proposal: University of North Carolina at Chapel Hill for its <br />Department of Family Medicine in the School of Medicine ("UNC") and Orange <br />County. <br />Services: The purpose of this contract is to purchase the services of the Orange <br />County Health Department to improve the quality of patient self-management through <br />an evidence-based American Diabetes Association (ADA) self-management & <br />medical nutrition therapy (MNT) program aimed specifically at helping to close the <br />gap in health disparities among minorities who have diabetes. <br />a. Amount: UNC would pay the County a total of $ 78,996; $19,586 in the current <br />fiscal year, and the remaining $59,410 in the next fiscal year. <br />b. Term: The contract will begin February 2012 and end on May 31, 2013. <br />FINANCIAL IMPACT: Financial impact is listed above. <br />RECOMMENDATION(S): The Manager recommends that the Board authorize the County <br />Manager to sign the above listed agreement and any amendments or renewals of that <br />Agreement. <br />
