Please complete the <br />following information, limited to the front and back of this form. Hawick Ct. <br />Chapel Hill, NC 27516 <br />Phone: 919-942-1657 <br />Primary County Staff Contact: <br />Janet Tyler, Director of Department Of Aging <br />2551 Homestead Rd Chapel Hill, NC 27516 <br />Phone: 919-968-2070 <br />How many times per month does this board/commission meet, including any <br />special meetings and sub-committee meetings? <br />Bi-monthly committee business meetings and quarterly subcommittee visits to long term <br />care facilities. <br />Brief Statement of Board/Commission's Assigned Charge and Responsibilities. <br />• Stay apprised of general conditions for those residing in long-term care facilities. <br />• Make official unannounced quarterly visits to each adult care home as well as <br />unannounced annual visits to family care homes. <br />• Prepare reports as required by the NC Division of Aging & Adult Services to report <br />concerns encountered and long-term care issues in general. (Reports are public <br />records available through the regional ombudsman.) <br />• Serve as a central point for fostering increased community involvement in long-term <br />care facilities. <br />• Promote community education and awareness of the needs of residents in long-term <br />care facilities. <br />
