Statement of Qualifications for Construction Material Testing Services Irerracon <br /> Central Efland/Northern Buckhorn Sewer Extension in Orange County,NC y <br /> September 21,2011 a Terracon Proposal No.P70110416 <br /> PROJECT MANAGEMENT APPROACH <br /> Signify Presence and Responsibilities at the job site. If required by the building official, <br /> Terracon personnel will sign in on the appropriate form posted with the building permit. <br /> Observe Assigned Work. Terracon personnel will inspect work for which they are responsible <br /> and aware of for conformance with the building department approved (stamped) plans and <br /> specifications and applicable provisions of the applicable North Carolina Building Code. <br /> Our testing and observation services will be scheduled by the Owner's or Contractor's <br /> representative and performed on a part-time, as-requested basis. This scheduling practice in <br /> itself will result in lower fees as compared to a full-time site technician. <br /> We propose to utilize the listed procedures during testing and observation of the following <br /> elements to ensure a high standard of quality control and efficiency: <br /> • Soils — The Terracon technician will perform soil density testing using nuclear density <br /> gauges. This will reduce site visit durations and result in lower fees. <br /> • Foundations & Cast-in-Place Concrete — The same Terracon technician performing <br /> foundation testing can also inspect the reinforcing steel and test the concrete, <br /> eliminating the need for multiple technician site visits. <br /> • Hot Mix Asphalt Paving —The Terracon technician will utilize a nuclear density gauge <br /> to test the compaction of in-place hot mix asphalt at the time of placement, eliminating <br /> the need for a follow-up coring visit. <br /> Fees for mileage and equipment (trip charges)will be included in the hourly personnel rates. <br /> Provide Qualified Personnel. Terracon will provide qualified personnel to perform the <br /> assigned work. Our proposed staff has the appropriate certifications including: [CC <br /> (International Code Council), NICET (National Institute for Certification of Engineering <br /> Technologies), ACI (American Concrete Institute), PTI (Post Tension Institute), NCDOT (North <br /> Carolina Department of Transportation). <br /> Report Nonconforming Items. Terracon personnel will bring nonconforming items to the <br /> immediate attention of the contractor. If any such item is not resolved in a timely manner or is <br /> about to be incorporated into the work, the design team, the owner and the building official will <br /> be notified immediately and the item noted in Terracon's written report. Terracon personnel will <br /> write a separate report to be posted at the job site, if requested, regarding noted discrepancies. <br /> The report will contain, as a minimum, the following information about each nonconforming item: <br /> • Description and exact location <br /> • Reference to applicable detail of approved plans/specification <br /> • Name and title of each individual notified and method of notification <br /> • Resolution or corrective action taken. <br /> In addition to communicating non-conformance issues immediately to the designated parties, <br /> any non-conformance item that cannot be corrected the same day will be recorded in a non- <br /> 10 <br />
