REVISION TO FIiWA-1273 CONCERNING PERSONAL INFORMATION ON <br />PAYROLL. SUBMISSIONS: <br />(1-20-09) SP1G59 <br />Revise the Standard Special Provision FHWA-1273 Required Contract Provisions Federal Aid <br />Construction Contracts as follows: • <br />Section V, Paragraph 2b is replaced with the following: <br />The payroll records shall contain the name, and the last four digits of the socialAsecurity number • a ~ • <br />of each such employee, his or her correct classification; hourly rates of wages paid (including <br />rates of contributions or costs anticipated for bona fide fringe benefits or cash equivalent thereof <br />the types described in Section 1(b)(2)(B) of the Davis Bacon Act); daily and weekly number of <br />hours worked; deductions made; and actual wages paid. <br />
