Contract No. TJCOG— CBS —004 <br />depending on the Work performed by the Contractor, in accordance with the specifications set forth in the <br />attachments described below. Report templates will be available on your internet portal. <br />1. Quarterly Progress. This report will document a comparison of actual accomplishments, the cost status, <br />schedule status, any changes in approach that have been approved by TJCOG, actual or anticipated <br />problems or delays and actions taken, any changes or absence of key personnel, and a description of <br />outreach/marketing activities. <br />2. Data Collection Site Requirements for Fuel Dispensing contains the instructions and specifications <br />applicable to support the installation and operation of data collection devices at fuel dispensing sites. To <br />the extent that the Contractor either operates a fuel dispensing site or provides infrastructure for a fuel <br />dispensing site, the Contractor shall comply with these specifications, which may be modified from time <br />to time as necessary to collect the required data. <br />3. Data Collection Site Requirements for Vehicle Fleets contains the instructions for the collection of <br />weekly operational data from the use of fleet vehicles. If the Contractor is a fleet operator under the <br />Contract, the Contractor shall provide the required data in the form and manner set forth in this <br />attachment. <br />Buy American Provisions <br />If applicable, the Contractor is required to provide a listing of the manufacturers of the equipment purchased to <br />perform activities funded by the Carolina Blue Skies & Green Jobs Initiative, in compliance with the Buy <br />American (see Attachment A -2) requirements. <br />Davis Bacon Provisions <br />All construction projects are required to comply with requirements in the Davis -Bacon Act. The provisions <br />referenced in Attachment A -3 relating to wage rates or the Davis -Bacon Act refer to local standards as <br />determined by the Secretary of Labor as they apply to this Contract. The Secretary of Labor has made the <br />determinations by county that apply to this contract. These determinations are incorporated by reference and <br />included herein as Attachment A -4. The Contractor shall apply the wage rates that apply to this Contract. <br />In order to comply with these provisions, all construction projects are required to submit Form WH347, Weekly <br />Wage reports. The Form shall be uploaded to your project's internet portal. Hard copies will not be accepted. <br />_Special Status Reports <br />A report is required (via email), as soon as possible, after any of the following events occur: <br />1. Developments that have a significant favorable impact on the project. <br />2. Problems, delays, or adverse conditions which materially impair the ability to meet the objectives of <br />the award or which may require the OEI or the U.S. DOE to respond to questions relating to such <br />events from the public. Report on any of the following incidents and include the anticipated impact <br />and remedial action to be taken to correct or resolve the problem /condition: <br />a. Any single fatality or injuries requiring hospitalization of five or more individuals. <br />b. Any significant environmental permit violation. <br />c. Any verbal or written Notice of Violation of any Environmental, Safety, and Health <br />statutes. <br />d. Any incident which causes a significant process or hazard control system_ failure. <br />e. Any event which is anticipated to cause a significant schedule slippage or cost increase. <br />£ Any damage to Government -owned equipment in excess of $50,000. <br />g. Any other incident that has the potential for high visibility in the media. <br />Final Report <br />Provide information for a final report due 60 days after the contract terminates on April 30, 2014. This <br />includes providing the Property Certification, including the required inventories of non - exempt property.
