act tt— 5 <br />G <br />Attachment 2: Community Based Public Health Initiative <br />Grant Project Ordinance <br />Program Number 11 -644 <br />Be it ordained by the Orange County Board of County Commissioners that pursuant to Section 13.2 of <br />Chapter 159 of the General Statutes of North Carolina, the following grant project is hereby adopted. <br />Section 1. The project authorized is the Community Health Advocacy Program (CHAP) as awarded to the <br />Orange County Health Department by the Kellogg Foundation. The purpose of this four year <br />grant is to improve the health of minority populations in targeted communities. <br />Section 2. The officers of the County are hereby directed to proceed with the grant project within the grant <br />document and the rules and regulations of the Kellogg Foundation. <br />Section 3. The following revenue is anticipated to be available to complete this project: <br />1992 -93 Grant Funds - CHAP <br />$52,820 <br />1993 -94 Grant Funds - CHAP <br />$93,884 <br />1994 -95 Grant Funds - CHAP <br />$93,626 <br />1995 -96 Grant Funds - CHAP <br />$82,160 <br />1996 -97 Grant Funds - CHAP <br />$33,750 <br />1997 -98 Grant Funds - CHAP <br />$11,250 <br />1998 -99 Grant Funds - Community Health Initiative <br />$30,028 <br />1999 -00 Grant Funds - Community Health Initiative <br />$20,463 <br />2000 -01 Grant Funds - Community Health Initiative <br />$46,717 <br />2001 -02 Grant Funds- Community Health Initiative <br />$44,056 <br />Total CHAP Grant Funds $508,754 <br />Section 4. The following amount is appropriated for this project: <br />Human Services - Community Based Public Health Initiative $508,754 <br />Section 5. The finance officer is hereby directed to maintain within the Grant Project sufficient specific <br />detailed accounting records to provide the accounting to the grantor agency required by the <br />grant agreement and federal and state regulations. <br />Section 6. Funds may be advanced from the General Fund for the purpose of making payments due. <br />Reimbursement requests should be made to the grantor agency in an orderly and timely <br />manner. <br />Section 7. Copies of this grant project ordinance shall be made available to the finance officer for <br />direction in carrying out this project. <br />Section 8.
