act tt— 5 G Attachment 2: Community Based Public Health Initiative Grant Project Ordinance Program Number 11 -644 Be it ordained by the Orange County Board of County Commissioners that pursuant to Section 13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is hereby adopted. Section 1. The project authorized is the Community Health Advocacy Program (CHAP) as awarded to the Orange County Health Department by the Kellogg Foundation. The purpose of this four year grant is to improve the health of minority populations in targeted communities. Section 2. The officers of the County are hereby directed to proceed with the grant project within the grant document and the rules and regulations of the Kellogg Foundation. Section 3. The following revenue is anticipated to be available to complete this project: 1992 -93 Grant Funds - CHAP $52,820 1993 -94 Grant Funds - CHAP $93,884 1994 -95 Grant Funds - CHAP $93,626 1995 -96 Grant Funds - CHAP $82,160 1996 -97 Grant Funds - CHAP $33,750 1997 -98 Grant Funds - CHAP $11,250 1998 -99 Grant Funds - Community Health Initiative $30,028 1999 -00 Grant Funds - Community Health Initiative $20,463 2000 -01 Grant Funds - Community Health Initiative $46,717 2001 -02 Grant Funds- Community Health Initiative $44,056 Total CHAP Grant Funds $508,754 Section 4. The following amount is appropriated for this project: Human Services - Community Based Public Health Initiative $508,754 Section 5. The finance officer is hereby directed to maintain within the Grant Project sufficient specific detailed accounting records to provide the accounting to the grantor agency required by the grant agreement and federal and state regulations. Section 6. Funds may be advanced from the General Fund for the purpose of making payments due. Reimbursement requests should be made to the grantor agency in an orderly and timely manner. Section 7. Copies of this grant project ordinance shall be made available to the finance officer for direction in carrying out this project. Section 8. Positions authorized through this grant project include:
