Please find enclosed your grant award check in the amount of <br />$2,500. The Period of Performance for this grant is July 15, 2003 through December 31, 2003. Yow <br />grant award number is CCCFY02S-0010. <br />Enclosed are the following documents: <br />• Two copies of the grant guidelines. Please sign and date one copy and return it to me at the <br />address provided in the guidelines. <br />• Copy of the programmatic and fiscal reporting forms.
