The <br />community service provider shall specify how the service needs oflow-income minority <br />elderly and rural elderly will be met through the services identified on the Provider <br />Services Summary (DOA-732). This narrative shall address outreach and service <br />delivery methodologies that will ensure that this target population is adequately served <br />and conform with specific objectives established by the Area Agency on Aging, for <br />providing services to low income minority individuals.
