20 <br />DOA-733 <br />(Rev. 2/03) <br />Home and Community Block Grant for Older Adults <br />County Funding Plan <br />July 1, 2003 through June 30, 2004 <br />Methodology to Address Service Needs of Low-Income Minority Elderly <br />(Older Americans Act, Section 306 (a)(5)(A)(ii) <br />Community service Provider: Joint Orange-Chatham Community Action <br />County: Orange <br />The Older Americans Act requires that that the service provider attempt to provide services to low- <br />Income minority individuals in accordance to their need for aging services. The community service <br />provider shall specify how the service needs of low-income minority elderly and rural elderly will be <br />met through the services identified on the Provider Services Summary (DOA-732). This narrative shall <br />address outreach and service methodologies that will ensure that this target population is adequately <br />served and conform with specific objectives established by the Area Agency on Aging, for providing <br />services to low-income minority individuals. Additional pages may be used as necessary. <br />Joint Orange-Chatham Community Action, Inc. (JOCCA) was established in 1966 and built on a <br />foundation of citizen participation with constituents from all wallcs of life: poor and rich; black and <br />white; public and non-public. The agency is dedicated to serving the needs of all very low and low- <br />income residents of Orange and Chatham counties. The Agency's mission is "to help people help <br />themselves" and attain a status of financial independence through self-help initiatives. JOCCA is a <br />vital part of lives of thousands of very low and low-income residents and plays a salient part in their <br />everyday existence.. To meet the needs of clients, JOCCA relies on the. coordinated efforts and <br />partnerships with other human service agencies, the private sector, local governments and private <br />foundations <br />
