4 <br />The financial impact in the first three years is limited to the provision of the in -kind contributions <br />of the County. After the grant period, staff will report to the Board of Commissioners on project <br />outcomes and successful interventions and seek the Board's decision as to whether or not the <br />program should be sustained through County support. <br />RECOMMENDATION(S): The Manager recommends that the Board: <br />• Approve receipt of the grant and approve the establishment of the Grant Project Ordinance <br />for a period of three years, <br />• Authorize a new grant- funded Senior Public Health Educator position for project <br />implementation with continuation of the position after the three -year grant- funded period <br />being contingent on Board of Commissioners decision, and <br />• Authorize the Board Chair to sign the grant agreement with the North Carolina Health and <br />Wellness Trust Commission. <br />
