27 <br />Teen Tobacco Use Grant Number: <br />GRANT AGREEMENT BETWEEN THE N.C. HEALTH AND WELLNESS TRUST FUND <br />COMMISSION AND COUNTY OF ORANGE <br />This Grant Agreement (also, "Agreement ") is entered into this 17th day of February <br />2003 ( "Effective Date ") by and between the North Carolina Health and Wellness Trust Fund <br />Commission ( "the Commission "), an agency of the State of North Carolina organized pursuant to <br />Article 6C of Chapter 147 of the N.C. General Statutes and, the Orange County Health <br />Department a unit of the County of Orange with its principal place of business at 300 West Tryon <br />Street, Hillsborough, North Carolina. (also, "the Grantee ") (also, together "the Parties" and each <br />in the singular "the Party ") <br />In consideration of mutual promises and such other valuable consideration as is set out in <br />this Agreement, the Parties do mutually agree to the following: <br />Section I. Term. <br />This Agreement shall commence on the Effective Date and shall terminate twelve months later <br />( "Termination Date ") unless sooner terminated pursuant to this Agreement. The Commission <br />may renew this agreement for two subsequent one -year terms if the Grantee receives satisfactory <br />program evaluations and if the Commission has sufficient funds to renew the Agreement. <br />Section II. Scope of the Grant. <br />A. The purpose of the Grant that is the subject of this Agreement is to provide a community <br />based prevention and cessation program for teenage North Carolina residents (the <br />"Project ") of Orange County. In performing this Project, the Grantee shall develop, <br />perform and complete the work set out in the Grantee's application for the Project which <br />is reproduced at Exhibit A and which is expressly incorporated by reference into and <br />made a part of this Agreement. <br />B. The Grantee shall further comply with all reporting requirements of this Agreement and <br />shall participate in a state -level outcomes study by complying with all reasonable <br />requests of Commission's selected outcomes evaluator(s). <br />
