Urange County HOME Consortium Response: The following are the strategies outlined in the CoC's 14-Year Plan to End Homelessness to reduce Chronic Homelessness. The grange County Consortium is adopting these strategies and incorporating them into its Consolidated Plan. • Strategy i.i: Establish an assertive street outreach program that targets unsheltered homeless people at natural gathering places throughout Orange County. • Strategy i.Z: Establish an outreach system in Northern grange County that uses the congregate feeding programs as a place to begin identifying those who are chronically homeless in the rural part of the county, • Strategy 1.3: Create an Assertive Community Treatment FACT} Team that targets those who are chronically homeless and ~ntegr~es the team with the above outreach efforts. • Strategy 1.4: Ensure that both inpatient !and outpatient substance abuse treatment is made available to thane Gk~ronically homeless in~vlduals who desire that service. If inpatient treatment is necessary, make sure that permanent housing is not lost during the ~nati~~t'stay. • Strategy 1.5: Identify strategies designed to address the needs for shelter and services for individuals w~t~ ~~complex behaviors that result in being banned from kitchen shelter services. • Strategy 1.fi: Sheltered chronically l~ameless !;people will be able to move into permanent ..hau~ing by receiving"the services necessary far them to obtain and mainta~~n permanent hou-sing. • Strategy iw7. 40 units will be rehabbed/rented built to provide permanent supportive haus~ng ~inclu~rng ;.the .u:se of Assertive Community Treatment Teams} for the ch~oni~ homeless ira''~range County within the first 3-5 years of the'~oian~ ::. • strategy 1.8: insure ~ha ,nonprofit developers have the organizational and financial capacity. ~~o create new housing units within the community for the chronically homeless. • Strategy 1.9R ~dentify a wide variety of sites for housing the chronically homeless throughout the county in the most fair and effective places within the countyy • Strategy 1.io: Establish a rigorous evaluation mechanism that measures the cost of individuals who are chronically homeless before and after they are receiving housing and support services, 4, Homelessness Prevention-The jurisdiction must describe its planned action steps over the next year to address the individual and families with children at imminent risk of becoming homeless.
