O~Zp-ac,o~l-11 ~ <br />l a_~ y~ ~!J o ~1 ATTACHMENT 3 <br />.S ~' <br />Senior Citizen Health Promotion Program <br />Grant Project Ordinance <br />Be it ordained by the Orange County Board of County Commissioners that pursuant to Section <br />13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is <br />hereby adopted. <br />Section 1. This project authorized is the Senior Citizen Health Promotion Program to develop <br />a comprehensive and coordinated County health promotion and disease prevention <br />program for citizens involving a partnership of public agencies, University Health <br />Affairs and private groups. <br />Section 2.The officers of the County -are hereby directed to proceed with the grant project in <br />accordance with the mutual cooperation agreement between the County and the <br />University of North Carolina at Chapel Hill, as approved by the Board of County <br />Commissioners on June 30, 1997. <br />Section 3.The following revenue is anticipated to be available to complete this project <br />Grant Funds (1994-95) <br />Grant Funds (1995-96) <br />Grant Funds (1996-97) <br />Grant Funds (1997-98) <br />Fees for Service (1997-98) <br />Grant Funds (1998-99) <br />Fees for Service (1998-99) <br />Grant Funds (1999-00) <br />Fees for Service (2000-01) <br />Grant Funds (2000-0 l ) <br />Grant Funds (2001-02) <br />Grant Funds (2002-03) <br />Fees for Service (2003-04) <br />Grant Funds (2003-04) <br />Fees for Service (2004-OS) <br />Grant Funds (2004-OS) <br />Total Revenue <br />Section 4.
