` ~~ la ~~/aooy <br />AN AGREEMENT BETWEEN <br />ORANGE COUNTY HEALTH DEPARTMENT <br />AS A PART OF THE COUNTY OF ORANGE <br />AND <br />TRIANGE J COUNCIL OF GOVERNMENTS <br />THIS AGREEMENT, effective November 1, 2004 through June 30, 2005 by and <br />between the Orange County Health Department, hereinafter referred to as OCHD and. Triangle J <br />Council of Governments, hereinafter referred to as TJCOG. The Orange County Health <br />Department is part of the County of Orange. <br />WITNESSETH: <br />WHEREAS, TJCOG was an active partner in the development of the Active Seniors <br />Project as submitted to the North Carolina Department of Health and Human Services by the <br />OCHD, and <br />WHEREAS, TJCOG and the OCHD agreed that the OCHD would act as the fiscal agent <br />for the Active Seniors Project in accordance with the requirements of the North Carolina <br />Department of Health and Human Services, Division of Public Health, and <br />WHEREAS, OCHD has been awarded one-time funding to carry out the elements of the <br />Active Seniors Project, and <br />WHEREAS, TJCOG desires to provide services as outlined in the Active Seniors Project <br />Proposal, under the terms and conditions set forth in this Agreement and consistent with the <br />intent authorized by the funds provided by the North Carolina Division of Public Health, <br />NOW, THEREFORE, in consideration of the premises and of the following mutual <br />promises, covenants, and conditions, the parties agree as follows: <br />1. TJCOG will perform the following activities in an effort to increase the opportunities <br />for physical activity for residents using or living in the areas around Homestead Park, <br />the Southern. Human Services Center and the anticipated Southern Orange Senior <br />Center: <br />• Conduct a review of all relevant local plans and prepare a summary document <br />of findings by January 2005. <br />• Prepare a GIS-based map of current and planned facilities and potential <br />physical activity sites in a radius to be determined around the planned <br />Southern Senior Center by January 2005. <br />• Participate as a project leader in the selection of an assessment tool and the <br />development of a community training to conduct the assessments in the <br />neighborhood surrounding the Southern Human Service Center by March <br />2005. <br />
