9 <br />Lead Organization (s): Department on Aging- Public Transportation Div. with Emergency <br />Management Services. <br />Accomplishments: 2010: <br />1. Interim Director participated in the Emergency Management planning process to update the <br />Emergency Operations Framework. <br />2. Aging Transitions staff assisted in planning for the use of companion animal mobile trailers for <br />use in conjunction with the emergency shelters. <br />Recommendations for 2011: <br />1. Combine the DSS Special Needs Registry, <br />Sheriff Dept's Project Lifesaver. <br />the Dept on Aging Emergency Call list and the <br />Objective C-3: Improve coordination of all public transit routes and services <br />within Orange County and the Triangle region. <br />Lead Organizations: Orange County Dept on Aging (OPT), Triangle Transit Authority <br />NC Dept of Transportation, and Transportation Service Board <br />Accomplishment: 2010: <br />1. Supported the transfer of Orange Public Transportation to the Planning Department in <br />anticipation of its eventual relocation to a larger transit system resulting in improved services for <br />older adults <br />Recommendation for 2011: <br />1. Monitor the services provided to older adult as the OPT transit system is transferred to Chapel <br />Hill Transit <br />Objective C-4: Expand mobility efforts through the use of companions, paid or~ <br />volunteer (drivers and escorts) for frail/elderly who require door through door service. <br />Lead Organization (s): Dept. Aging with Partners -O.P.T. RSVP and A Helping Hand <br />Recommendation for 2011: <br />1. Recruit volunteers to serve as mobility aides for frail older adults accessing public <br />transportation. <br />Goal D: Transitional Care- Promote the transition and maintenance of older persons in the <br />most appropriate health care provider setting. <br />Objective D-1: Maintain older persons in the most appropriate setting through the <br />development or expansion of innovative models of aging-friendly community <br />programs. <br />6 <br />
