12 <br />Recommendations for 2011: <br />1. Continue funding of public location information sessions supported by MIPPA and LIS <br />funding available through the Community Resource Connections (CRC) <br />2. Continue search for resources for the Mood, Memory and Mobility Clinic suspended for lack <br />of funding in June 2010 <br />3. Make potential clients aware of availability of the PACE services offered through Piedmont <br />Health Services.. <br />3. Focus: Older Adults with Disabilities/Moderate Impairment <br />Goal F: Older Community Residents with Disabilities/Moderate Impairment -Maximize <br />the safety, functional ability, and quality of life for impaired, community-dwelling older <br />persons and their family caregivers. <br />Objective F-1-: Increase community recognition, support, education and <br />empowerment of family caregivers. <br />Lead Organization (s): Department on Aging- Aging Transitions Division <br />Accomplishments:2010: <br />1 Recruited a group of 20 African Americans to attend the Duke Alzheimer's conference with <br />the goals of creating dementia volunteers in multiple churches. <br />2. Assisted Jewish Family Services with development of its new community center <br />Recommendations for 2011: <br />1. Continue Caregiver Day Out community respite program with expansion to year-round <br />service. <br />2. Continue dementia outreach project to Afro-American churches. <br />3. Continue to offer five or more support groups and numerous community education <br />presentations on caregiving. <br />Objective F-2: Offer best practices in mental health care for older persons in <br />affordable, stigma-free, non-psychiatric settings. <br />Lead Organization(s): Department on Aging- Aging Transitions Div. with UNC <br />Geropsychiatry, OPC Mental Health and Faith Communities <br />Accomplishments: 2010: <br />1. Continued offering support groups for people with mild dementia and concurrent <br />depression/adjustment issues. <br />2. Provided consultations as requested to NC Access re: coordination of mental health care in a <br />stigma-free setting. <br />3. Established the Seymour Center as a host location for a weekly Alcoholics Anonymous <br />meeting. <br />4. Continued to co-sponsor with the National Alliance on Mental Illness (NAMI) an on-going <br />series of mental health education/support workshops at the Seymour Center <br />9 <br />
