Healthy Carolinians Program v ~ ,~ and ~- ~~7 <br />Grant Project Ordinance Lr, - a 3 - z va ~/ <br />~~ <br />Be it ordained by the Orange County Board of County Commissioners that pursuant to Section <br />13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is <br />hereby adopted. <br />Section 1.The project authorized is the Healthy Carolinians Program in the Orange County <br />Health Department. The purpose of this grant program is to provide a plan to guide <br />and assist the County in planning and implementing health care strategies to promote <br />healthy lifestyles, improve health status and prevent premature death and injury for al! <br />residents in the County regardless of age, race, income or education. <br />Section 2.The officers of the County are hereby directed to proceed with the grant project within <br />the grant document and the rules and regulations of the grantors. <br />Section 3.The following revenue is anticipated to be available to complete this project: <br />1994-95 Grant Funds -CHAP $93,626 <br />1995-96 Grant Funds -CHAP $82,160 <br />1996-97 Grant Funds -CHAP $33,750 <br />1997-98 Grant Funds -CHAP $11,250 <br />1998-99 Grant Funds -Community Health Initiative $30,028 <br />1999-00 Grant Funds -Community Health Initiative $20,463 <br />2000-01 Grant Funds -Community Health Initiative $46,717 <br />2001-02 Grant Funds- Community Health Initiative $49,056 <br />2002-03 Grant Funds -Community Health Initiative $51,575 <br />2003-04 Grant Funds -Community Health Initiative $25,000 <br />2004-05 Grant Funds -Community Health Initiative $15,000 <br />Total Grant Funds $237, 839 <br />2003-04 Transfer from General Fund $890 <br />2004-OS Transfer from General Fund $10,910 <br />Total Revenue $249, 639 <br />Section 4. The following amount is appropriated for this project: <br />Human Services -Community Based Public Health Initiative $249, 639 <br />Section 5. The finance officer is hereby directed to maintain within the Grant Project sufficient <br />specific detailed accounting records to provide the accounting to the grantor agency <br />required by the. grant agreement and federal and state regulations. <br />
