O <br />Exercise Equipment Addendum <br />Page 2 <br />Each employee who will use or contribute to the cost of the equipment must sign the Affidavit. <br />Affidavit <br />Any equipment purchased with grant funds becomes County property. 1 understand that any <br />contribution I may make towards the purchase of exercise equipment is non - refundable. I am <br />aware of the risks associated with using the equipment and will use it only as intended, keeping <br />my safety and that of my co- workers in mind.
