~o <br />~~: r <br />Memorandum of Understanding between the <br />North Carolina Department of Health and Human Services <br />Community Justice Partnership Program <br />And the <br />North Carolina Department of Gorrections <br />(OMM} <br />,~~- <br />The Offender Management Model (OMM} is a joint effort between the Community Justice <br />Partnership Program (CJPP}, Department of Corrections (DOC); Division of Community <br />Corrections (DCC.}, Division of Alcoholism and Chemical Dependency programs (DACDP) and <br />Department of Health and Numan Services (DHMIDDISAS). OMM embodies the partnership <br />between these agencies. DCC and Treatment Altematives to Street Crimes (TASC) are committed <br />to provide treatment and control of high risk/high needs offenders under supervision in the <br />communitya DACDP is committed to providing treatment to offenders participating in DACDP <br />programs`. Utilizing principles of effective interventions, this partnership between the D.G~c-,~1PP <br />and DHHS, will promote consistent matching of treatment and supervision levels for quality <br />offender management. . <br />TASC, Probation Services, DACDP and CJPP agree to: <br />A. Develop a unified caselservice plan specific to the offender's needs in order to <br />accomplish mutually agreed goals. <br />B. Promote an open exchange of communications and exchange information accordance <br />with- rules of confidentiality and legs! waivers: <br />C. Abide by and promote a comprehensive assessment, case plan development, and routine <br />case staffing. <br />D. To ensure participation in follow-up process meetings on a frequent basis as well as ongoing <br />Management. and maintenance. <br />E. To strive toward a point where the local systems are integrated and work together <br />seamlessly, serving the offenders and promoting public safety. <br />Tres#ment Alternatives to Street Crimes (TASC) mission is to provide clinics! assessment, - <br />treatment matching, referrals and care management services to offenders, <br />TASC agrees to; `~' ~' <br />A. Screen and clinically assess ail appropriate referrals <br />8. Match offender's needs with appropriate treatment services, paying special attention to <br />- responsive issues _ <br />- G: Make the appropriate service referrals <br />
