ATTACHMENT 5 <br /> Healthy Carolinians Program <br /> Grant Project Ordinance <br /> Be it ordained by the Orange County Board of County Commissioners that pursuant to Section <br /> 13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is <br /> hereby adopted. <br /> Section 1.The project authorized is the Healthy Carolinians Program in the Orange County <br /> Health Department. The purpose of this grant program is to provide a plan to guide <br /> and assist the County in planning and implementing health care strategies to promote <br /> healthy lifestyles, improve health status and prevent premature death and injury for all <br /> residents in the County regardless of age, race, income or education. <br /> Section 2.The officers of the County are hereby directed to proceed with the grant project within <br /> the grant document and the rules and regulations of the grantors. <br /> Section 3.The following revenue is anticipated to be available to complete this project: <br /> 1998-99 Grant Funds - Community Health Initiative $30,028 <br /> 1999-00 Grant Funds - Community Health Initiative $20,463 <br /> 2000-01 Grant Funds - Community Health Initiative $46,717 <br /> 2001-02 Grant Funds- Community Health Initiative $49,056 <br /> 2002-03 Grant Funds - Community Health Initiative $51,575 <br /> 2003-04 Grant Funds - Cate B. Reynolds Foundation $25,000 <br /> 2004-05 Grant Funds- Cate B. Reynolds Foundation $15,000 <br /> 2005-06 Grant Funds- Cate B.Reynolds Foundation $10,000 <br /> 2005-06 Grant Funds-UNC Health Initiative (AHEC) $800 <br /> Total Grant Funds $248,639 <br /> 2003-04 Transfer from General Fund $890 <br /> 2004-05 Transfer from General Fund $10,910 <br /> 2005-06 Transfer from General Fund $4,818 <br /> Total Revenue $265,257 <br /> Section 4. The following amount is appropriated for this project: <br /> Human Services- Community Based Public Health Initiative $265,257 <br /> Section 5. The finance officer is hereby directed to maintain within the Grant Project sufficient <br /> specific detailed accounting records to provide the accounting to the grantor agency <br />
