ATTACHMENT 5 Healthy Carolinians Program Grant Project Ordinance Be it ordained by the Orange County Board of County Commissioners that pursuant to Section 13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is hereby adopted. Section 1.The project authorized is the Healthy Carolinians Program in the Orange County Health Department. The purpose of this grant program is to provide a plan to guide and assist the County in planning and implementing health care strategies to promote healthy lifestyles, improve health status and prevent premature death and injury for all residents in the County regardless of age, race, income or education. Section 2.The officers of the County are hereby directed to proceed with the grant project within the grant document and the rules and regulations of the grantors. Section 3.The following revenue is anticipated to be available to complete this project: 1998-99 Grant Funds - Community Health Initiative $30,028 1999-00 Grant Funds - Community Health Initiative $20,463 2000-01 Grant Funds - Community Health Initiative $46,717 2001-02 Grant Funds- Community Health Initiative $49,056 2002-03 Grant Funds - Community Health Initiative $51,575 2003-04 Grant Funds - Cate B.Reynolds Foundation $25,000 2004-05 Grant Funds - Cate B. Reynolds Foundation $15,000 . 2005-06 Grant Funds - Cate B.Reynolds Foundation $10,000 2005-06 Grant Funds -UNC Health Initiative (AHEC) $800 Total Grant Funds $248,639 2003-04 Transfer from General Fund $890 2004-05 Transfer from General Fund $10,910 2005-06 Transfer from General Fund $4,818 Total Revenue $265,257 Section 4. The following amount is appropriated for this project: Human Services- Community Based Public Health Initiative $265,257 Section 5. The finance officer is hereby directed to maintain within the Grant Project sufficient specific detailed accounting records to provide the accounting to the grantor agency
