<br />i <br />STATE OF NORTH CAROLINA <br />COUNTY OF WAKE <br />NORTH CAROLINA <br />DEPARTMENT OF TRANSPORTATION <br />HUMAN SERVICE TRANSPORTATION <br />MANAGEMENT GRANT AGREEMENT FOR <br />PUBLIC BODY ORGANIZATIONS <br />(FOR COMMUNITY TRANSPORTATION <br />SYSTEMS OPERATING IN URBAN COUNTIES) <br />DOT PROGRAM NUMBER: DOT-03 <br />and <br />ORANGE COUNTY <br />PROJECT NUMBER: 07-US-056 <br />WBS ELEMENT: 36221.19.5.1 <br />PO NUMBER: 5700000112 <br />THIS AGREEMENT made this they day of mom, 20~, (hereinafter referred to as <br />AGREEMENT) by and between the DEPARTMENT OF TRANSPORTATION (hereinafter <br />referred to as "Department", an agency of the State of North Carolina) and Orange County <br />(acting in its capacity as the designated recipient of Human Service Transportation <br />Management program funds, hereinafter referred to as the "Contractor"). <br />WHEREAS, Article 2B of Chapter 136 of the North Carolina General Statutes designated <br />the Department of Transportation as the agency of the State of North Carolina responsible for <br />administering funding assistance for public transportation in rural and small urban areas by way <br />of a formula grant program to be administered by the State; and <br />WHEREAS, the purposes of Article 2B of Chapter 136 are to enhance access of people in <br />nonurbanized areas for purposes such as health care, shopping, education, recreation, public <br />services, and employment by encouraging the maintenance, development, improvement, and <br />use of passenger transportation systems; and <br />WHEREAS, the Contractor has been designated as the recipient of Human Service <br />Transportation Management (for Community Transportation Systems operating in urban <br />counties) funds, and <br />WHEREAS, Article 2B of Chapter 136 of the North Carolina General Statutes designated <br />the Department of Transportation as the agency of the State of North Carolina responsible for <br />Project 07-US-056 <br />Orange County <br />Page 1 of 49 <br />
