STATE OF NORTH CAROLINA UNCH#64q <br /> COUNTY OF ORANGE <br /> AGREEMENT BETWEEN <br /> THE UNIVERSITY OF NORTH CAROLINA HOSPITALS <br /> AND <br /> ORANGE COUNTY <br /> THIS AGREEMENT, made effective the 1st day of July, 1997 by and between <br /> The University of North Carolina Hospitals, hereinafter referred to as "The Hospital," and <br /> Orange County, for and on behalf of the Department of Aging, hereinafter referred to as <br /> "The County"- <br /> WITNESSETH: <br /> WHEREAS, The County desires to provide the services of a full-time Wellness <br /> Program Director to work with The Hospital to expand existing programs and to develop <br /> and implement new programs to serve the health and wellness needs of seniors in Orange <br /> County,North Carolina; and <br /> WHEREAS, The Hospital desires to provide financial and other resources to The <br /> County in support of existing and new senior health and wellness programs under the <br /> terms and conditions set forth in this Agreement. <br /> it <br /> NOW, THEREFORE, in consideration of the premises and of the following mutual <br /> promises, covenants, and conditions, The Hospital and The County agree as follows: <br /> 1. The County will provide the following: <br /> a. The services of a full-time Wellness Program Coordinator who will have <br /> the following responsibilities: <br /> (1)Prepare an annual Older Adult Wellness Plan("Wellness Plan") <br /> with the assistance of the Wellness staff team submitted to the Advisory Committee for <br /> approval. <br /> (2) Implement the Wellness Plan which includes the coordination of <br /> community-based partnership resources and the direct provision of services offered at <br /> Senior Centers. <br /> (3)Promote the increased involvement of older adults in the <br /> Wellness Program with the assistance of community-based partnerships. <br /> (4)Evaluate the impact of the Wellness Program initiatives on older <br /> adults by the establishment of assessment procedures. <br />
