(OMM} is a joint effort between the Community Justice <br />Partnership Program (CJPP), Department of Corrections (DCC}, Division of Community <br />Corrections (DCC}, D'nviSifln of Alcoholism and Chemical Dependency Programs (DACDP} and <br />Department of Health and Human Services (DHMrDDISAS). OMM embodies the partnership <br />between these agencies. DCC and Tr~eatment.4lternafives to Street Crimes (TASC} are committed <br />to provide treatment and. control of high riskltligh needs offenders under supervision in the <br />community. DACDP is commitked to providing treatment to offenders .participating in DACDP <br />_ programs. Utilizing principles of efi~ctive interventions, this partnership between the DCC, CJPP <br />and DHHS, will promote consistent matching of treatment and supervision levels far quality <br />offender management. ~ - <br />TASC, Probation Services, DACDP ar3d CJPP agree to: <br />A. Develop a unified case/service plan specific to ~e of€ender's needs in order to <br />accomplish mutually aged c~oa's: <br />~. To strive toward a point when the focal systems are integrated and work Together . <br />seamlessly, serving the dfrenders and promoting public safaty. <br />Treatment Alternatives to Street Climes (TASC) mission is to provide clinical assessment, <br />treatment matching, referrals and care management services to offenders. <br />TASC agrees to: <br />A. SCreAn and. clinically assess all appropriate referrals . <br />B. lvlatch offender's needs vtirruh appropriate treatment services, paying SpGCial attention to <br />i~sponsive issues <br />C. Make the appropriate SerVlre referrals <br />
