H. Commission Amendments -Any amendments to the Application made by the Commission shall be <br />set forth in Attachment A-2. <br />I. Contract Performance Period Extension -For grant monies that remain after the final Termination <br />Date for the project period established by the Request for Grant Proposals, the Commission may <br />approve an additional Grant Cycle to use any remaining balance for further progress of the Grantee's <br />Project. <br />J. Effective Date -The date the initial Grant Cycle begins, as designated by the Commission. <br />K. Financial Report Formats- Grantee's Monthly Expense Report(s) and Annual Certification of Account <br />Balance, in the form set forth in Attachment E-I and E-2, respectively. <br />L. Governmental Entities - A state agency and a local government or other political subdivision of the <br />State or a combination of such entities. A list of local (non-stateagencies) Governmental Entities may <br />be found at www.ncauditor.net. <br />M. Grant -Total amount approved by the Commission for Grantee's Project and set forth in Attachment <br />B-1. <br />N. Grant Cycle -Each period of up to twelve (12) months for which the Commission approves funding. <br />O. Grantee Evaluation and Development Provider (GEDP~ The entity(ies) or person(s) designated by the <br />Commission to provide annual needs assessment and action planning, Regular performance and <br />budget monitoring and evaluation, and coordinate delivery of Technical Assistance and training. <br />P. Grant Program or Program -The specific initiative which Grantee's Project is designed to support.. <br />The Program addressed by this Project Grant is described in the Grantee's Grant Application in <br />Attachment A-l, Commission Approved Amendments to the Grantee's Grant Application in <br />Attachment A-2, Minimum Requirements in Attachment A-3, and Action Plan in Attachment A-4. <br />Q. NC Health and Wellness Trust Fund Commission (Commission) -The governmental entity that <br />provides funding for the Project(s) in support of Commission Program(s) as established by N.C. <br />General Statute § 147-86.30 et seq. and is governed by Chapter 10 of the North Carolina <br />Administrative Code. <br />R. Non-Governmental Entities - Anon-profit corporation which has a significant purpose promoting the <br />public's health. A list of local Governmental Entities may be found at www.ncauditor.net. <br />S. Outcome Evaluator(s) (OE) - Agency(ies) and/or entity(ies) used by the Commission to collect, <br />analyze and report on the effectiveness ofthe overall Program including the individual Projects funded <br />under the Program. <br />T. Performance Obligations -The specific items the Grantee agrees to perform as set forth in this <br />Agreement and Attachment A. Attachment A includes the Grantee's Application in Response to the <br />Commission's Request for Grant Proposal,- any Commission Approved Amendments to the Grant <br />Application, any other minimum requirements or deliverables that Grantee must perform in order to <br />receive and maintain the Grant, and an Action Plan. <br />U. Project -The Program services set forth in Attachment A which the Grantee agrees to implement. <br />V. Reports -All required documents as defined in this Agreement and as may be requested from time to <br />time by the Commission with respect to the Project and the Grant. <br />6 <br />
