Health and Wellness Trust Fund Commission <br />Application for Teen Tobacco Use Prevention and Cessation Initiative: <br />Community/School Prevention Program or Special Proiect Grant <br />Instructions to Applicants <br />Only one proposal from each applicant organization will be accepted. All applicants <br />must submit a complete written application by the deadline identified in the Timeline section <br />of the 1tFP in order to be considered for a grant award. Applicants are strongly encouraged to <br />use the Microsoft Word-based online application found at www.hwtfc.or~, comprised of <br />Parts I, II and III of the Application. Applicants also need to complete and submit the <br />Microsoft Excel-based budget format (referenced in Part III of the Application and available <br />on the HWTFC website, www.hwtfc.or~). In addition, for applications to be considered <br />complete, the Additional Materials listed in Part IV are mandatory and must be sent by mail <br />or delivery service in one package, postmarked by March 29, 2005. <br />This application consists of Part I (Standard HWTFC Grant Application), Part II (Proposed <br />Project), Part III (Proposed Budget and Fiscal Information) and Part IV (Additional <br />Materials). <br />The Application is available either as an MS/Word document (.doc file) or an Adobe <br />document (.pdf file). Applications must be submitted via email to hwtfc e,ncmail.net by 5:00 <br />PM, March 29, 2005. The title of the email and all attached documents must start with the <br />lead applicant organization's Federal ID number and name (e.g. 123454321 Wright <br />Hospital.doc). <br />It is recommended that you save the online document on your local network or computer <br />hard drive and continue to save it until it has been completed and submitted. This saved <br />copy is for your internal records. <br />Enter your responses to application. questions in the highlighted spaces provided in each <br />section by double-clicking on a field and tabbing between the questions. Please be concise in <br />your response (fixed character limits are used). There is a free text section at the end of Part I <br />where you can provide additional information for clarification. <br />When complete, submit to HWTFC via email attachment at hwtfc(a~ncmail.net by 5:00 PM, <br />Mazch 29, 2005. The Additional Materials listed in Part IV aze mandatory and must be sent <br />by mail or delivery service in one package, postmarked by March 29, 2005. <br />
