ATTACHMENT 0 <br />Healthy Carolinians Program <br />Grant Project Ordinance <br />Be it ordained by the Orange County Board of County Commissioners that pursuant to Section <br />13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is <br />hereby adopted. <br />Section 1.The project authorized is the Healthy Carolinians Program in the Orange County <br />Health Department. The purpose of this grant program is to provide a plan to guide <br />and assist the County in planning and implementing health care strategies to promote <br />healthy lifestyles, improve health status and prevent premature death and injury for all <br />residents in the County regardless of age, race, income or education. <br />Section 2.The officers of the County are hereby directed to proceed with the grant project within <br />the grant document and the rules and regulations of the grantors. <br />Section 3.The following revenue is anticipated to be available to complete this project: <br />1998 -99 Grant Funds.- Community Health Initiative <br />$30,028 <br />1999 -00 Grant Funds - Community Health Initiative <br />$20,463 <br />2000 -01 Grant Funds - Community Health Initiative <br />$46,717 <br />2001 -02 Grant Funds- Community Health Initiative <br />$49,056 <br />2002 -03 Grant Funds - Community Health Initiative <br />$51,575 <br />2003 -04 Grant Funds - Cate B. Reynolds Foundation <br />$25,000 <br />2004 -05 Grant Funds - Cate B. Reynolds Foundation <br />$15,000 <br />2005 -06 Grant Funds - Cate B. Reynolds Foundation <br />$10,000 <br />2005 -06 Grant Funds - UNC Health Initiative (AHEC) <br />$800 <br />2005 -06 Grant Funds - NC Department of Public Health <br />$7,000 <br />Total Grant Funds <br />$255,639 <br />2003 -04 Transfer from General Fund <br />$890 <br />2004 -05 Transfer from General Fund <br />$10,910 <br />2005 -06 Transfer from General Fund <br />$4,818 <br />Total Revenue $272,257 <br />Section 4.
