Contract#68-2012 <br /> CNC Access,Inc.dba ResCare HomeCare <br /> (8) Business Associate Addendum(Attachment I) YES <br /> (9) Certification Regarding Transportation(Attachment J) YES <br /> (10) Outcomes &Reporting(Attachment I) YES <br /> 2. _Precedence AmonLy Contract Documents: In the event of a conflict between or among <br /> the terms of the Contract Documents and this Agreement, the terms of this Agreement shall <br /> control. In the event of a conflict between or among the terms of the Contract Documents,then <br /> the Contract Documents with the highest relative precedence shall prevail. The order of <br /> precedence shall be the order of documents as listed in Section 1, above,with Attachment A <br /> having precedence over Attachment B and so forth. If there are multiple Contract Amendments, <br /> the most recent amendment shall have the highest precedence and the oldest amendment shall <br /> have the lowest precedence. <br /> 3. Effective Period: This Agreement shall be effective from July 1, 2010 through June 30, <br /> 2011. <br /> 4. Contractor's Duties: The Contractor shall provide the services to the County described <br /> in Attachment B in accordance with the approved rate as described in Attachment B, Scope of <br /> Work, and shall meet the requirements set forth in Attachment N, Outcomes and Reporting. The <br /> Contractor further agrees that the County may require the removal or replacement of an <br /> employee upon reasonable suspicion that the employee has violated the terms of this contract, <br /> including but not limited to Attachment C and Attachment J. <br /> S. County's s Duties: The County shall pay the Contractor in the manner and in the <br /> amounts specified in the Contract Documents. <br /> (a) The total amount paid by the County to the Contractor under this Agreement for <br /> the provision of services to the Department of Social Services shall not exceed: $415,647. This <br /> amount consists of$415,647 in Federal, State and County funds (CFDA# ), <br /> $0 (source of other funds if applicable). <br /> (b) The total amount paid by the County to the Contractor under this Agreement for <br /> the provision of services to the Department on Aging shall not exceed: $50,000. This amount <br /> consists of$50,000 in Federal, State and County funds(CFDA# ), $0 <br /> (source of other funds if applicable). <br /> [X] (c) There are no matching requirements from the Contractor under this Agreement for <br /> the provision of services to the Department of Social Services. <br /> [ ] (d) The Contractor's matching requirement is$ ,which shall consist of <br /> [ ] In-kind [ ] Cash <br /> [ ] Cash and In-kind [ ] Cash and/or In-kind <br />
