Details are provided in the Emergency Preparedness <br />Grant Program Frequently Asked Questions..} <br />• A list of most current board of directors <br />• Financial document as required based on the organization's size <br />- Certified public accounting audit for organizations with $500,000 or more in total <br />annual revenue <br />- Finanaal review by an independent certified public accountant for organizations with <br />at least $100,000, but less than $500,000 in total annual revenue <br />- Financial statement certified as accurate and complete by the organization's chief <br />executive officer for organizations with annual revenues less than $100,000 <br />- Government entities do not need to submit the full audit for their organization.
