Details are provided in the Emergency Preparedness <br />Grant Program Frequently Asked Questions..} <br />• A list of most current board of directors <br />• Financial document as required based on the organization's size <br />- Certified public accounting audit for organizations with $500,000 or more in total <br />annual revenue <br />- Finanaal review by an independent certified public accountant for organizations with <br />at least $100,000, but less than $500,000 in total annual revenue <br />- Financial statement certified as accurate and complete by the organization's chief <br />executive officer for organizations with annual revenues less than $100,000 <br />- Government entities do not need to submit the full audit for their organization. Given <br />the large size of the audits, these entities are required to submit the following four <br />components of the audit: <br />1 } auditor's report (cover letter) <br />2) balance sheet <br />3) statement of revenues and expenditures <br />4) auditor's notes <br />• Prior fiscal year financial statements showing actual expenses and sources of <br />income <br />• Current fiscal year approved budget showing actual expenses and sources of <br />income year to date <br />• Up to three letters of support (optional) <br />Signature: Date: <br />Name of Project Director: Jack Ball <br />Title: Director, Department of Emergency Management <br />Signature: Date: <br />
