<br />When developing a project, it is important to start by clearly identifying the objectives of the project, the strategies to achieve the
<br />objectives, and the expected outcomes. The BCBSNC Foundation uses this information when making funding decisions because it helps
<br />to ensure the effectiveness of the projects. This worksheet is intended to clearly define your project's objectives, activities, and outcomes.
<br />Instructions:
<br />1. Determine the objectives of the project. These should be broad statements about how the project will address the barriers and
<br />opportunities described in the project description. Objectives should be limited to one or two per project.
<br />2. Identify the activities to be implemented to achieve the objectives.
<br />3. Identify short-term outcomes for the project that could be achieved within the firs# 12 months of funding. These outcomes
<br />should be SMART {specific, measurable, achievable/realistic, relevant, and time-bound).
<br />4. Identify the method of datalinformation collection to be used to determine the results of the project's short-term outcomes.
<br />5. Identify long-term outcomes for the project and specify the timeframe for achieving those outcomes. These outcomes should be
<br />SMART (specific, measurable, achievable/realistic, relevant and time-bound).

<br />While you are being asked to report the information requested above in a table format, this information is not meant to be linear
<br />(i.e., there will not be a short-term outcome for every activity). However, for every short-term outcome, there should be a method of
