~o <br />. _.. __ _ <br />.. . ~-_ <br />_ . <br />Please respond to each of the following statements (four-page maximum for the entire narrative}. <br />Structure the narrative according to the four sections indicated below; title each section with the <br />corresponding project heading. <br />PROJECT SUMMARY (suggested length is one page) <br />Section should include the following information: <br />• Statement of need (including applicable natural disaster-related statistics) <br />• Target population and geographic area served <br />• Description of project and how it addresses a vulnerable population <br />• Expected outcomes <br />• Project budget <br />• Amount requested <br />PROJECT DESCRIRTION <br />Section should include more detailed information on the following: <br />• Statement of need {including applicable natural disaster-related statistics) <br />• Target population (including applicable demographic statistics of vulnerable population(s) to <br />be served by the project) <br />• Description of project and haw it addresses an unmet need with regard to emergency <br />preparedness for vulnerable populations <br />• Project components <br />IMPLEMENTATION AND COORDINATION <br />Section should include more detailed information on the following: <br />• Provide a schedule of implementation for meeting the project objectives <br />• Describe any service currently available in your community that is similar to your proposed <br />project. Explain how the services are coordinated or complement one another. <br />• State whether the project requires collaboration with other organizations to accomplish its <br />objectives, If so, describe the responsibilities of each partner organization. <br />SUSTAINABILITY <br />Section should include the following information: <br />• Plan for sustainability if the project is to be continued beyond this grant <br />• If the total grant request is for equipment, the applicant must demonstrate that funding is <br />available for training, implementation, maintenance, etc. <br />• As a reminder, preference will be given to proposals that suggest activities, plans or <br />equipment that can be sustained over the long-term, versus being used during the next natural <br />disaster and then discarded {i:.e.., used one-time only). <br />
