Brown <br />Relationship~tle of Authorized Representative: Chair, Board of County Commissioners <br />BY ENDORSING THIS SIGNATURE PAGE, Margaret W. Brown (naive) declares <br />that he or she is duly authorized by the Applicant to make the certifications and assurances on behalf of <br />the Applicant and bind the Applicant to comply with them. Thus, when its authorized representative <br />signs this document, the Applicant agrees to comply with all Federal statutes, regulations, executive <br />orders, and administrative guidance required for any North Carolina Department of Transportation <br />(NCDOT) statewide CTP application~in which its projects are included and submitted to the Federal <br />Transit Administratiov (FTA) during Federal Fiscal Year 1998. <br />FTA and NCDOT intend that the certifications and assurances the Applicant selects on this form, as <br />representative of the certifications and assurances in Appendix L, should apply, as rtquined, to each <br />project for which the Applicant seeks FTA assistance through NCDOT now, or may later, seek FTA <br />assistance through NCDOT during federal fiscal year 1'998. <br />The Applicant affirms the truthfirlness and accuracy of the certifications and assurances it has made in the <br />statements submitted herein with this document and any other submission made to NCDOT for inclusion <br />in a submission to FTA, and acknowledges that the provisions of the Program Fraud Civil Remedies Act <br />of 1986, 31 U. S. C. 3801 et seg,;, as implemented by USDOT regulations, "Program Fraud Civil <br />Remedies," 49 CFR Part 31 apply to any certification, assurance or submission made to FTA. The <br />criminal fraud provisions of 18 U. S. C. 1001 apply to any certification, assurance, or submission made <br />in connection with the F'fA formula assistance program for urbanized areas, and may apply to any other <br />certification, assurance, or submission made in connection with any program administered by FTA. <br />In signing this document, I declare under penalties of perjury that the foregoing certifications and <br />assurances, and any other statements made by me on behalf of the Applicant are true and correct. <br />per: January 20, 1998 <br />Authorized Representative of <br />Applicant <br />FY99 APPLICATION NCDOT-PTD <br />
