PROFESSIONAL SERI/~CES AGREEMENT <br />ORANGE COUNTY PARTNERSHIP FOR YOUNG CHILDREN ~OCPYC) <br />AND <br />EMERGENCY MEDICAL SERVICES (EMS) <br />. t=Y 19976 <br />AgrQentent 9 SS-9784-003 <br />Pwpose: Tt1® Orange County Partnership for Young Children and EMS agree to <br />enter a limited services agreement for the purpose of developing the wreleo~me to <br />the IMo~id baby project. <br />Agreement Period: The agreement period begins June 1,1998 and oondudes <br />September 3,1998. This agreement may be cancebd by any party with atwo- <br />week written notice. <br />Agreement Total: The tote! oust of this agreement is not to exceed 55.000. a0 <br />that includes all cost for the it~ns identified by EMS, inducting first aid felts, <br />poison control materials, safety magazine, smoke detectors and batteries <br />Rsspor~aubes: <br />Orange County Partnershie for Young Children agrees to: <br />1. Reimburse EMS for ttte cysts far services and activities as described below <br />under Contractor respor~sibitides. <br />7. Monitor the Contractor for compliance with the teems at this oontr~td. <br />3. Specify aft r'oports required from the Contractor. <br />Contractor agrees to' <br />1. Work collaboratively with OCPYC to develop W{e/GOrrre to the V4~orid Baby <br />project and to identity and purdl8se safety equipment and supplies as <br />dsscxibed. <br />2_ Maintain records of receipts and disbursements and such books and records <br />vnN be msde availsble to the OCPYC or its representative upon request. <br />3. Submit itemized budget dertaifing equipment purchases. Any item over 5500 <br />wiN be returned to the Partnership should the activity be discontinued for any <br />reason. <br />4. Attend scheduled meetings with OCPYC as requested and subrrltt the <br />following reports fa this agreement: <br />• Data Report <br />• Cash and to-kind match tog <br />Any other roports spec;ifred by OCPYC <br />5. Identify Smart Start and OCPYC as the funding source for the contracted <br />projects to all promotions and Literature. <br />6. These funds wiN be used ordy for the purpose speciricelly set forth in this <br />agreement. Any portion of the funds not used ter these purposes wilt be <br />repaid to OCPYC <br />EMS.doc <br />
