The evaluation of [he program will be completed by [he Extension Agent, Nutrition <br />Programs Supervisor. A comprehensive computerized reporting system has been in <br />use by EFNEP for years. Breas[feeding reporting forms currently in use <br />(Attachment I) will be adapted to meet program evaluation needs. Demographic <br />data and measurements of breastfeeding initiation and duration rates among <br />population groups will be collected. Information will be available at any point, but <br />wil I be formally assessed quarterly [o monitor [he progress of [he program.
