FY 2010-2011 <br />Addendum A to <br />Agreement between UNC and OCHD <br />For Medical Director Services <br />This Addendum to the Medical Services. Agreement ( "Agreement "), dated July 1, 2010, between <br />Orange County, North Carolina a body politic and corporate of the State of North Carolina <br />( "County ") on behalf of the Orange County Health Department ( "OCHD ") and the University of <br />North Carolina at Chapel Hill ( "Provider" or "University ") on behalf of its Department of Family <br />Medicine in the School of Medicine, ( "Department "), is for Medical Director Services to the <br />Chapel Hill - Carrboro City Schools ( "CHCCS). This Addendum to the Agreement, which is <br />hereby fully incorporated by reference, is entered into by and between the County, the Provider <br />and the CHCCS. <br />WITNESSETH: <br />Whereas, the Chapel Hill- Carrboro City Schools (CHCCS) employs one (1) School <br />Nurse who is a licensed and qualified Nurse Practitioner (Head Start Program), and; <br />Whereas, CHCCS seeks to have the School Nurse provide limited nurse practitioner <br />services to students at her site, and; <br />Whereas, the School Nurse Practitioner requires medical supervision, and; <br />Whereas, the Orange County Health Department (OCHD) maintains a Memorandum of <br />Agreement with CHCCS to provide medical direction to school nurses by the OCHD Medical <br />Director, and; <br />Whereas, The University of North Carolina at Chapel Hill (`The University') through its <br />Department of Family Medicine of the UNC School of Medicine (Department) provides medical <br />supervision to OCHD Nurse Practitioners through its Medical Director contract and is agreeable <br />to expanding the contract to provide medical supervision to the CHCCS School Nurse <br />Practitioner. <br />Now, therefore, for the consideration herein named, the parties agree to the following: <br />1. Scope of Services: <br />a. The OCHD Medical Director (or other mutually agreed upon UNC Family Practice <br />physician) will provide medical supervision to the CHCCS School Nurse <br />Practitioner from August 15, 2010 to June 15, 2011 (10 months) in accordance with <br />the terms and conditions of the Agreement, which is hereby incorporated by <br />reference and this Agreement is amended thereto. <br />b. Insurance. CHCCS will maintain medical malpractice insurance equal to or <br />exceeding $1 million per incident and $3 million upper limit coverage for its <br />
