3 <br />If Orange County Emergency Services provided these same transport services, it would <br />conservatively require an increase in EMS staff by no less than six transport units. Based on <br />current costs, the following investment would be needed: <br />• 31 Emergency Medical Technicians (EMTs): Current annual cost (including benefits) per <br />employee is approximately $53,300, or a personnel services cost of $1,652,000 annually. <br />• 8 Ambulances: The benchmark for reserve units is one backup unit for every two <br />frontline units. Using a conservative ratio of one to three, eight units would be required to <br />provide sufficient primary and backup fleet support. At approximately $225,000 per unit <br />and amortized over five years, the annual cost for units would be $360,000. <br />• Rent, fuel, medical supplies, and maintenance: This is a difficult cost to estimate, but the <br />best assumption is $450,000 annually. <br />• Training and Associated Support: Approximately $17,890 annually <br />• Business/Operational Contract Management (1 full time equivalent staff position): <br />Estimated at $60,000 per year <br />Based on the costs estimates above, the total annual cost for Orange County to provide non- <br />emergency transport services similar to those provided by JAS would be approximately <br />$2,539,000. <br />RECOMMENDATION(S): The Manager recommends that the Board: <br />1. Approve on Second Reading the granting of a franchise for a five year term to <br />Johnston Ambulance Service, Inc. for Basic Life Support Convalescent Transport <br />Services and Emergency Services. <br />a. Convalescent Transport Services <br />• Basic Life Support Transport Services <br />b. Emergency Services <br />• Basic Life Support Transport Services <br />2. Authorize the Chair to sign the Franchise Agreement; <br />3. Approve the Franchise Agreement under which Johnston Ambulance Service, Inc. will <br />operate during the five year period of franchise, and that as a condition of the <br />Franchise, JAS enter into an Operations Agreement with Orange County within 90 days <br />to provide for the daily operational functions of JAS as it works within the Orange <br />County Emergency Services System; and <br />4. Authorize the Emergency Services Director to negotiate the Operations Agreement with <br />JAS and the County Manager to sign the Agreement after review by the County <br />Attorney or his staff. <br />
