AMENDMENT TO THE CONSOLIDATED CONTRACT FOR 1994-95 TO EJCTEND THE <br />EARLY PERIODIC SCREENING DIAGNOSTIC TESTING (EPSDT) PROJECT <br />The Board accepted a $10,000 grant from the Department of <br />Environment, Health and Natural Resources to provide "bridge" funding to <br />continue the EPSDT Outreach program (Health Check) and two permanent, time- <br />limited positions associated with this program through September 30, 1994 and <br />authorized the Chair to sign the contract amendment and the budget. <br />C. EBTENSION OF PERMANENT, TIME-LIMITED MATERNITY CARE COORDINATION <br />POSITION <br />The Board extended an existing, time-limited, Maternity Care <br />Coordination position for one additional year. This position will provide <br />case management services for an average monthly caseload of 36 prenatal <br />patients covered by Medicaid and an average monthly caseload of 20 child <br />service coordination clients in order to reduce poor birth outcomes. <br />D. MILL POINTE PRELIMINARY PLAN <br />The Board approved the Preliminary Plan for Mill Pointe subject to <br />the conditions contained in the Resolution of Approval as attached to these <br />minutes on pages <br />E. HICKORY HILL PRELIMINARY PLAN <br />The Board approved the Preliminary Plan for Hickory Hill Sub- <br />division, including item B7, subject to the conditions contained in the <br />Resolution of Approval as attached to these minutes on pages <br />
