~-1~ -~/~ <br />y~ <br />2010-2011 FREEDOM HOUSE RECOVERY CENTER AGREEMENT <br />This Agreement, made and entered into the _ day of March 2010, by and between the <br />County of Orange, a political subdivision of the State of North Carolina, 200 South <br />Cameron Street, Hillsborough, North Carolina, 27278, ("County") and Freedom House <br />Recovery Center, Inc, ("FH"), anot-for-profit corporation, located at 104 New Stateside <br />Drive, Chapel Hill, North Carolina 27516. ("Provider") <br />WITNESSETH: <br />WHEREAS, it is in the interests of the County that said program be assisted by the County <br />and thereby enhance its availability to residents of the County, and said program addresses <br />an important community human services need, as identified by the Board of Commissioners; <br />and <br />WHEREAS, the North Carolina Department of Correction Criminal Justice Partnership <br />Program ("CJPP") has awarded a North Carolina Department of Correction Criminal Justice <br />Partnership Program ("NCDOCCJPP") grant to Orange and Chatham Counties, North <br />Carolina; and <br />WHEREAS, the aforementioned Counties desire to use some portion of the NCDOCCJPP <br />grant to contract with FH to provide a continuum of care through a Resource Center for <br />individuals in the intermediate punishment population in Orange County and Chatham <br />County; and <br />WHEREAS, the FH Resource Center ("FHRC") desires to provide a continuum of care for <br />eligible Intermediate sanctioned offenders including Intensive Outpatient services, Detox, <br />Residential Placement, and Cognitive Behavior Intervention (CBI) Services. <br />NOW, THEREFORE, in consideration of the above and the mutual covenants and <br />conditions hereafter set forth, the County and FH agree as follows: <br />1. Term of the Agreement. The term of this Agreement shall be a program year <br />beginning July 1, 2010 to June 30, 2011. <br />2. Scope of Services. <br />a. Provider will provide services, as outlined in the attached Criminal Justice <br />Partnership Program Application for Continuation of Implementation Funding <br />and any amendments or revision thereto which is attached as Exhibit "A" and <br />incorporated by reference, as well as the services provided below to the CJPP- <br />eligible intermediate punishment offender population in Orange County and <br />Chatham County. The Scope of Services may be different from the original <br />application based on State appropriation; however, any revisions or amendments <br />to this Agreement must be approved in writing by the County and attached to <br />this Agreement. <br />
